3 Tips to Eliminating Bootcamp Back Pain

Bootcamps are great.

Back pain is not great.

When I ran bootcamps last summer, injuries and pain was a big thing.

I was always having to modify exercises or provide alternate exercises for bootcampers with a previous injury or pain.

I wanted to share a few tips on back pain and bootcamp exercises.

Heavy Kids Backpacks Bad for Kid's Backs

Heavily loaded backpacks can lead to lumbar spine asymmetry during loading
Researchers reported that 8-kg backpacks led to disc compression and up to a 10° change in Cobb’s angle.

TORONTO — Investigators who conducted the first study using MRI to assess how lumbar disc height is affected when subjects wear heavy backpacks found that caudal intervertebral discs compressed the most relative to typical school backpack loads.

“Lumbar asymmetry was a new and unexpected finding of this study,” said Timothy Neuschwander, MD, of San Diego. The study subjects — three boys and five girls with a mean age of 11 years — had normal Cobb angles when not wearing backpacks, but those angles increased to almost 10° when they wore backpacks weighing 8 kg.

Little Arthritis in the Facet Joints


Facet Joint Osteoarthritis and Low Back Pain in the Community-Based
Population

To evaluate the association between lumbar spine facet joint osteoarthritis
(FJ OA) identified by multidetector computed tomography (CT) and low back
pain (LBP) in the community-based Framingham Heart Study

Results: CT imaging revealed a high prevalence of FJ OA (59.6% of males and
66.7% of females). Prevalence of FJ OA increases with age. By decade, FJ OA
was present in 24.0% of <40-years-olds,>70-years-olds. By spinal level the prevalence of FJ OA was: 15.1% at L2-L3,
30.6% at L3-L4, 45.1% at L4-L5, and 38.2% at L5-S1. In this community-based
population, individuals with FJ OA at any spinal level showed no association
with LBP.

Conclusion: There is a high prevalence of FJ OA in the community. Prevalence
of FJ OA increases with age with the highest prevalence at the L4-L5 spinal
level. At low spinal levels women have a higher prevalence of lumbar FJ OA
than men. In the present study, we failed to find an association between FJ
OA, identified by multidetector CT, at any spinal level and LBP in a
community-based study population.

Exercise is Good After Back Surgery

Exercise Rehab Appears Useful and Safe After Lumbar Disc Surgery

AMSTERDAM, Oct. 10 -- After surgery for a prolapsed lumbar disc, exercise appears to be a beneficial part of a rehabilitation program, according to a systematic review of the literature.

A review of 14 randomized controlled trials showed that performing some type of exercise after surgery was associated with significant improvements in pain and functional capability in the short term, without increasing the risk of another operation, Raymond Ostelo, Ph.D., of VU University Medical Center here, and colleagues reported in a Cochrane Review.

NSAID and Low Back Pain

This is an interesting study commenting on anti-inflammatory drugs and low back pain.

It looks like they are beneficial for the short term.

Nonsteroidal Anti-Inflammatory Drugs for Low Back Pain: An Updated Cochrane Review

Abstract

Study Design. A systematic review of randomized controlled trials.

Objectives. To assess the effects of nonsteroidal anti-inflammatory drugs (NSAIDs) and COX-2 inhibitors in the treatment of nonspecific low back pain and to assess which type of NSAID is most effective.

Summary of Background Data. NSAIDs are the most frequently prescribed medications worldwide and are widely used for patients with low back pain. Selective COX-2 inhibitors are currently available and used for patients with low back pain.

Methods. We searched the MEDLINE and EMBASE databases and the Cochrane Central Register of Controlled Trials up to and including June 2007 if reported in English, Dutch, or German. We also screened references given in relevant reviews and identified trials. Randomized trials and double-blind controlled trials of NSAIDs in nonspecific low back pain with or without sciatica were included.Results. In total, 65 trials (total number of patients = 11,237) were included in this review. Twenty-eight trials (42%) were considered high quality. Statistically significant effects were found in favor of NSAIDs compared with placebo, but at the cost of statistically significant more side effects. There is moderate evidence that NSAIDs are not more effective than paracetamol for acute low back pain, but paracetamol had fewer side effects. There is moderate evidence that NSAIDs are not more effective than other drugs for acute low back pain. There is strong evidence that various types of NSAIDs, including COX-2 NSAIDs, are equally effective for acute low back pain. COX-2 NSAIDs had statistically significantly fewer side effects than traditional NSAIDs.

Conclusion. The evidence from the 65 trials included in this review suggests that NSAIDs are effective for short-term symptomatic relief in patients with acute and chronic low back pain without sciatica. However, effect sizes are small. Furthermore, there does not seem to be a specific type of NSAID, which is clearly more effective than others. The selective COX-2 inhibitors showed fewer side effects compared with traditional NSAIDs in the randomized controlled trials included in this review. However, recent studies have shown that COX-2 inhibitors are associated with increased cardiovascular risks in specific patient populations.

Surrey Personal Trainer Reports Posture Training Decrease Low Back Pain

This is a great research summary. It shows that a postural technique of the Alexander Technique can help with low back pain. This is very exciting for clients as the Alexander Technique is something clients can do on their own to decrease their low back pain.

Alexander Educational Technique May Effectively Reduce Low Back Pain

An educational technique known as the Alexander technique, along with exercise, is effective vs massage for long-term relief of low back pain, according to the results of a factorial randomized trial reported in the August 20 Online First issue of the BMJ.

"Lessons in the Alexander technique offer an individualised approach designed to develop lifelong skills for self care that help people recognise, understand, and avoid poor habits affecting postural tone and neuromuscular coordination," write Paul Little, from the University of Southampton, Aldermoor Health Centre, Southampton, United Kingdom, and colleagues. "Lessons involve continuous personalised assessment of the individual patterns of habitual musculoskeletal use when stationary and in movement; paying particular attention to release of unwanted head, neck, and spinal muscle tension, guided by verbal instruction and hand contact, allowing decompression of the spine; help and feedback from hand contact and verbal instruction to improve musculoskeletal use when stationary and in movement; and spending time between lessons practicing and applying the technique."
